Form of the Beast

At the beginning of your upkeep, you may deal
damage to target player equal to the number of
creatures you control.
At the beginning of each end step, your life total
becomes 5.
Sacrifice Form of the Beast: Target creature you
control gets +X/+X where X is the number of
creatures you control. You lose the game at the
end of turn.
