Hell Hound Cloak

Until broken by Remove Curse or similar magic, you keep the cloak within reach at all times. {bold}Action{/bold}. Command the cloak to transform you into a hell hound for up to 1 hour (functions as Polymorph). You can revert to your normal form as a bonus action. {bold}Slipping Identity{/bold}. On the 6th use and every use after, make a DC 15 Cha save. {bold}On a failure{/bold}, the transformation lasts until dispelled or you drop to 0 HP, and you can’t willingly revert. If you remain a hell hound for 6 hours, the change becomes permanent. Your statistics become a Hell Hound’s and you lose your sense of self. Only Remove Curse or similar magic can restore you. If you remain permanently transformed for 6 days, only Wish can reverse it.
