At the beginning of your upkeep, put a pain counter of Lord of Pain, then Lord of Pain deals damage equal to the number of pain counters on it to you.
{b}{b},{t}: Put a pain counter on Lord of Pain.
{t}, remove all pain counters from Lord of Pain: Lord of Pain deals damage to target creature or player equal to the number of pain counters removed this way.
