Every time The Penitent One would be die in combat, instead place a -2/-1 and Guilt counter on it.
When The penitent one's toughness equals 0, remove all guilt counters, dealing X damage to The Penitent One's Controller and exiling X target nonland permanents where X equals the number of Guilt counters removed this way.
