Whenever a player draws a card, they reveal it. If it's a creature, put a token onto the battlefield under their control that's a copy of it, except it's a Nightmare Horror in addition to its other types. Otherwise, put three 0/1 Nightmare Illusion tokens onto the battlefield under their control. Either way, they then lose life equal to the number of cards in their hand plus the number of Nightmares they control.
